PART II: PATIENT SERVICE AGREEMENT

5. Establishing a Patient-Provider Relationship

A formal patient-provider relationship is established only after you have:

Completed a comprehensive, in-person medical evaluation with one of our Virginia-licensed healthcare providers at one of our physical clinic locations.

Provided a complete and accurate medical history.

Signed all required patient consent forms.

Been formally accepted as a patient by the Practice.

Use of this Website, email correspondence, or phone calls does not in any way constitute a patient-provider relationship.

6. Scope of Services and Service Area

Our services are strictly limited to individuals who are at least 18 years of age and are physically present within the Commonwealth of Virginia for all medical consultations and treatments. We are a specialty practice focused on men’s health and wellness and do not provide primary care or emergency services.

7. Telehealth and Electronic Communications

While all initial consultations and prescribing decisions occur in person, we may use electronic communication (such as secure patient portals or email) for administrative and follow-up purposes with our established patients. You understand and agree that:
